The final report provides information to Authority on your organization's activities related to the grant during the grant period, information about the youth involved in the program funded by the grant, and an accounting of how you spent the grant money. At the mandatory Grant Agreement Workshop, record-keeping is discussed. Grantees are provided with forms in MSWord and MSExcel that can be used to keep track of expenses, participants, and other things.

It is crucial that you have a tracking process and an organization bank account in place before you receive your grant money. If you don't have proper documentation for a particular expense, the Authority will request that that amount be returned to the Authority.
The Grant Agreement is specific about which line items and particular items, according to your application, are funded; you may not decide on your own to spend the funds on something else. If you need to change the agreed-on project budget, a request must be submitted in writing or by email to the Authority. The Board will then grant or not grant the requested change.
When are final reports due and where should they be turned in?
The Grant Agreement specifies when your final report is due, usually a month after your program ends. There is a slot in the door for after-hours; the building is open until 8 p.m.

Instructions for Completing the Sports Authority Final Report Form
The Final Report MUST BE TYPED.
IMPORTANT: The MSWord form can be used only with Word 2000 or later. Otherwise, you can complete the printed .pdf form, or request a printed copy from the Planning Council.
If the Word form is usable for you, save it to a working directory.
This form permits you to enter information ONLY in the designated spaces — these can be seen as underlined spaces. You can navigate through the document using your tab key or your mouse.
You will notice that, when you open the document, the first field is already selected. If you look at the status bar (at the bottom left of your screen), you will see notes that will help you to answer the questions. You can also press your F1 key to get help for each field. Most of the notes tell you how many characters and spaces that field will accept.
Be sure to answer all the questions.
Do not submit original receipts, just copies. Be sure to "white out" any credit card information.
