Submit the original and 7 copies. If you have not satisfactorily completed all due final reports or owe money to the Milwaukee Youth Sports Authority, you may not apply for funding. Click here to go to the GRANTS page and become familiar with the requirements and procedures. The application is at the bottom of the GRANTS page.
